Susquehanna Nursing & Rehabilitation Center has been recognized as a recipient of the Bronze-Commitment to Quality Award for its dedication to improving the lives of residents through quality of care. The award is the presented by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). Applicants who receive the Award are able to describe their mission, characteristics, key challenges and to recognize the relationship of these factors to the best of their ability to achieve performance improvement. Susquehanna provides short-term, comprehensive care for residents recovering from cardiac conditions such as Congestive Heart Failure (CHF), heart attack, and cardiac surgery. Our state recognized, award winning program feature an interdisciplinary team approach to cardiac recovery.
Our goal at Susquehanna is to maximize functional abilities and enhance quality of life for the stroke victim. Rehabilitation that helps restore a stroke survivor’s occupational, speech, and physical functions through cognitive and rehabilitative support services. Our therapy helps restore the critical ability of communication, restoring swallowing and eating abilities, as well as improving impairments in cognition such as judgment, reasoning, and problem solving in addition to helping the resident meet their highest level of independence.

With the goal of length of stay below national average and the highest standard in satisfaction surveys we are committed to meeting your individual rehab goals and expectations.
Susquehanna in affiliation with the University of Rochester offers telemedicine conferences for our Parkinson’s and neuromuscular disease residents.
Comprehensive Skin and Wound Care
Our interdisciplinary skin team is committed to the promotion and maintenance of resident skin integrity. Residents are assessed upon admission (routinely thereafter) and individual treatment plans are initiated. A team of licensed professionals specializing in wound care treatment compliments our staff. Our high-tech treatment approach utilizes specialized mattresses and in house debridement. Wound Vac (KCI) therapy is offered on site.
Alzheimer’s and Memory Impaired Disorders
Susquehanna provides care for people with memory disorders through our Special Care Program. Residents of this innovative care program enjoy of full range of personalized services to enhance each resident’s unique need for physical, cognitive, and emotional well-being.
Palliative, Comfort, and Hospice Care
As one approaches the end of life Susquehanna’s commitment to both the individual and their families is apparent. We believe that this time should be comforting and serene time with an assurance of dignity and respect. Where one can spend the last few days of life holding a hand and comforting one another. Upcoming plans show our further commitment, as our care team plans on offering these individuals private rooms with overnight accommodations for loved ones.
Long Term Care, Home Health Care, & Golden Days Adult Day Care
Living with dignity and satisfaction is ensured with Susquehanna's long term care likewise for services provided in the home for personal care, nursing, and therapy. We also have a facility for adult day care providing social services, activities, nursing, and therapy. Riverview apartments is our independent senior living housing unit adjacent to our Nursing Center.

Our nursing center is for residents in need of short term rehabilitative services and well as those in need of long term skilled nursing services. We place our emphasis on living.
